In order to facilitate the uniform review of proposals, as well as to assist proposing units with the fullest possible development of their programmatic strategies, many forms, templates and other proposal instruments have been developed. - Pre-approval Form: Graduate Concentrations and Certificates
Prior to the development of a full-blown concentration or certificate proposal, the Office of the Executive Vice President and Provost of the University wants to insure that all new concentrations and certificate initiatives be reviewed and implemented strategically from a university perspective. Please refer to the Office of the Provost Curriculum Development website for additional information regarding the Concentration and Certificate Pre-Approval Form.

Use this form if you are proposing to create a new concentration under
an existing degree. Please note that a "concentration" is a
subspecialty within a degree and major which reflects that the student
has fulfilled a designated, specialized course of study.

This form is for units proposing a graduate certificate. Please note that this form is for graduate certificates only.
- Changes to Existing Graduate Academic Programs

This form should be used when a unit is proposing a change to an already-approved academic program. These changes might include changes to the minimum required hours for a degree, a change to the culminating experience option(s) available to students in a program, a change in delivery format for a degree program, or the establishment of a dual or concurrent degree program.

These guidelines describe the policies and guidelines for the development and implementation of an accelerated bachelor/master's degree program, when both the bachelor's and master's degrees are existing programs.
- Proposal to Establish a Master's in Passing (MIP) - No Existing Master's Degree

This form can only be used when proposing to establish a Master's degree in Passing (MIP) under a PhD program that does not have a standalone master's degree in the same major as the PhD. The proposed major for the MIP must be identical to the major of the corresponding PhD program.
