Graduate Admission Requirements Change	Academic units use this form to propose changes to the admissions requirements for an existing graduate program.
Graduate Program Change Form	Academic units use this form to propose changes to the program requirements for an existing graduate program.
Application Review Terms Changes	Academic units use this form to propose changes to the application terms open for an existing graduate program.
Graduate Track Proposal Form	Academic units use this form to propose a new track under an existing graduate program.
Establishing a New Graduate Certificate	Academic units develop a full proposal to establish a new graduate certificate using the Proposal to Establish a New Program form in Kuali Curriculum Management (CM).
Establishing a New Graduate Concentration	Academic units develop a full proposal to establish a new concentration under an existing degree using the Proposal to Establish a New Program form in Kuali CM.
Establishing a New Graduate Degree	Academic units develop a full proposal to establish a new master's or a doctoral degree using the Proposal to Establish a New Program form in Kuali CM.
Individual Student Committee Approval Request	Academic units use this form to request approval for a faculty member to serve on a specific student’s committee in an area in which they do not already have graduate faculty status.
5-Year Program Committee Approval Request	Academic units use this form to request a 5-year approval for a faculty member to serve on a committee in an area in which they do not already have graduate faculty status.
