Committee Approval Request: Individual Student CommitteeUse this form for one-time approval to serve on a student committee.
Delaying Publication of Thesis/Dissertation (Embargo Status)Submitt this form to delay publication of your thesis/dissertation.
Maintain Continuous Enrollment: Master's and CertificateUse this form to request up to two semesters of non-registration. Request must be submitted to, and approved by your academic unit and Graduate College before the anticipated semester of non-registration.
Maintain Continuous Enrollment: PhDUse this form to request up to two semesters of non-registration. Request must be submitted to, and approved by your academic unit and Graduate College before the anticipated semester of non-registration.
Masters in PassingUse this form to request a Masters in Passing.
Petition to Graduate CollegeUse this form to petition for special exceptions to policy or procedure.
